Job summary

Patriot Healthcare is seeking a Hospice Certified Nursing Assistant to provide personal care to assigned patients according to nursing assessments and care plans, under the guidance of nurse supervisors.

You will participate with the interdisciplinary group (IDG) to develop and implement care plans, document tasks, and maintain a comfortable, dignified patient environment while adhering to hospice policies and procedures.

Responsibilities

• Participates with the IDG in developing and implementing the patient/family care plan.
• Performs tasks per Hospice policies and procedures and under supervision.
• Maintains patient comfort and dignity during care.
• Communicates with family and facility staff to ensure continued nursing services.
• Documents tasks and observations from each visit.
• Creates a warm, calm environment and supports patient well-being.
