Job summary

Patriot Healthcare is seeking a Hospice Certified Nursing Assistant (CNA) to provide personal care to assigned patients in accordance with nursing assessments and the care plan, under the supervision of nurses.

The role includes participating with the IDG in care planning, performing tasks per hospice policies, maintaining patient comfort, and assisting with daily living activities. Strong communication with families and staff is essential.

• Stewards the philosophy/mission of Patriot Healthcare focusing on human dignity in terminal and chronic illness care.
• Participates with the IDG in developing and implementing the patient/family care plan.
• Performs tasks per hospice policies and supervisor directions.
• Ensures patient comfort and dignity during all care activities.
• Documents tasks and communicates with family or facility staff to maintain care.
