Job summary

Affinity Health Management is seeking a qualified hospice aide to support patient care under the supervision of an RN. You will perform personal care, assist with daily living activities, monitor patient condition, and document observations in the EMR according to the plan of care.

The role requires completion of state-approved CNA/NAC training, CPR certification, and a high school diploma or GED. Prior hospice or long-term care experience is preferred, and driver’s license with insured vehicle

Qualifications

• Completion of a state-approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC) credential.
• Current Healthcare Provider CPR certification (hands-on demonstration required).

Responsibilities

• Provide personal care and report changes to RN Case Manager promptly.
• Communicate with patient and family using respectful, confidential care.
• Ensure safety and proper use of assistive devices and equipment.
• Assist with bathing, dressing, feeding, and activities of daily living.
• Maintain cleanliness and order in the patient’s living area and report concerns.
