Job summary

Reserve Care is seeking a dedicated Certified Nursing Assistant to provide personal care under RN supervision, supporting patients in home care settings in Ohio. You will collaborate with a transdisciplinary team, assist with activities of daily living, and help maintain a safe, comforting care environment while documenting care activities and observations.

Qualifications

• Must be able to read and write English proficiently.
• State Tested Nursing Assistant in good standing.
• Equivalent education/experience may substitute for minimum qualifications when legal requirements exist.

Responsibilities

• Provide personal care under the supervision of a nurse according to the patient’s plan of care.
• Collaborate with a transdisciplinary team in various settings.
• Assist with ADLs, provide safe environment, and support ROM and mobility as directed by the nurse.
• Record observations in the clinical record and complete charting and timesheets as required.
