Affinity Health Management is seeking a qualified Certified Nursing Assistant to support hospice patients under RN supervision. The role focuses on personal care, comfort, and timely communication with patients and families.

You will document observations and participate in care planning to ensure quality, continuity, and safety of care for terminally ill patients. The position requires a CNA license, high school diploma or GED, and the ability to read and follow instructions.

Qualificações

• Completion of state-approved CNA course and current license.
• Ability to read and follow written instructions and document care given.
• Understanding hospice philosophy and comfort in caring for terminally ill patients.
• Prior hospice/home health experience preferred.
• Basic computer skills to document in EMR.

Responsabilidades

• Perform personal care tasks as assigned promptly.
• Provide compassionate communication with patient and family while maintaining confidentiality.
• Ensure patient safety and proper use of equipment.
• Assist with bathing, grooming, feeding, and mobility as directed.
• Monitor skin condition and report changes to RN Case Manager.
• Record observations and care provided in the EMR as required.
• Support in transfers and range-of-motion exercises.
• Assist with self-administered medications as allowed by state regulations.
