Job summary

Affinity Health Management seeks a Certified Nursing Assistant for hospice care under the supervision of a registered nurse. You will perform personal care, document care, and report observations to ensure patient comfort and safety.

Requirements include PA CNA certification, high school diploma or GED, and prior hospice or home health experience preferred. You may assist with meds as allowed and must be vaccinated. Strong communication and compassion are essential.

Qualifications

• Complete state-approved CNA course with current license.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Understand hospice philosophy and comfort providing hospice care to the terminally ill.
• Prior hospice/home health experience preferred (1 year).
• Basic EMR documentation skills.
• At least 18 years of age.
• Licensed driver with insured vehicle in good working order.
• Must be fully vaccinated.

Responsibilities

• Provide personal care and attend to patient requests promptly.
• Communicate with patients and families with confidentiality and respect.
• Ensure safety and proper use of equipment as directed by RN.
• Assist with bathing, grooming, and changing bed linens.
• Assist in dressing and feeding as assigned.
• Support self-administered medications as allowed by regulation.
• Report pain or changes to the RN Case Manager.
