Job summary

Affinity Hospice Management LLC is seeking a Certified Nursing Assistant hospice aide to provide direct patient care under RN supervision. You will perform personal care, assist with activities of daily living, and document observations according to the patient’s plan of care.

The role emphasizes compassionate communication, safety, and adherence to hospice policies. Requires VA CNA certification, 18+ years old, a driving license, and vaccination as required by policy.

Qualifications

• State-approved CNA course completion and current license.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Understanding hospice philosophy and comfort with terminally ill patients.
• 1 year previous home health/hospice experience preferred.
• Basic computer skills to document in EMR.
• Age 18 or older.
• Valid driver’s license with insured vehicle.
• Must be fully vaccinated.

Responsibilities

• Perform personal care tasks as assigned and respond promptly to patient requests.
• Provide compassionate communication with patient and family while maintaining confidentiality.
• Ensure patient safety and use equipment correctly (foot stools, side rails, O2).
• Assist with bathing, grooming, oral hygiene, hair care, and linen changes.
• Assist with dressing, undressing, and feeding as directed.
• Avoid giving homemade foods or baked items prepared by the aide.
• Monitor skin and report any signs of breakdown to the RN.
• Report pain or changes in condition to the RN Case Manager.
• Assist in ambulation and prescribed exercises as directed.
• Provide ROM and transfers/positioning per instruction.
• Assist with self-administered medications as allowed by state rules.
• Assist with bedpans, urinals, light laundry, and light homemaker tasks.
• Perform ROM and simple procedures as directed with competency.
• Provide respite for patient/family when on-site, as appropriate.
• Keep living area clean and orderly per standards.
• Follow documentation and care procedures; participate in QA activities.
• Respect patient and family environment and personal needs.
• Report non-compliance to VA Dept. of Health within 14 days.
