Job summary

Affinity Health Management in Pittsburgh, PA is seeking a dedicated Hospice Aide (CNA) to support patient comfort and personal care under RN supervision.

You will perform direct care tasks, document observations, and assist families while adhering to care plans and safety standards. A PA CNA license is required, with priority given to candidates with hospice experience and a patient-first approach.

Qualifications

• Completion of state approved certified nursing assistant course with current license.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and provides specialized care to the terminally ill.
• 1 year home health/hospice experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle.
• Must be fully vaccinated.

Responsibilities

• Perform personal care needs after patient assignment and respond promptly.
• Communicate with patient and family with confidentiality and respect.
• Ensure safety and use of equipment as directed.
• Assist with baths, oral hygiene, grooming, and linen changes.
• Assist in dressing and undressing patients as assigned.
• Assist feeding; refrain from homemade foods.
• Monitor skin condition and prevent tissue breakdown.
• Report pain or changes to RN Case Manager.
• Assist with ambulation and supervised exercise.
• Provide ROM and transfers as directed.
• Assist with self-administered meds as allowed.
• Assist with bedpans, urinals, and light housekeeping.
• Perform range of motion and simple procedures as ordered.
• Provide respite to patient/family when on-site.
• Keep living area clean and orderly.
• Follow documentation and care procedures and professional conduct.
• Participate in quality assessment and performance improvement.
• Respect patient and family environment and needs.
• Report non-compliance to VA Dept. of Health within 14 days.
• Report suspected abuse or neglect to appropriate authorities.
