Job summary

UnitedHealth Group, in partnership with Amedisys, seeks a compassionate CNA Hospice Aide to deliver hands-on care to patients at home and support families during end-of-life care.

You will work closely with the hospice team to promote comfort, dignity and emotional well‑being, following the care plan, documenting observations, and performing routine personal care tasks in a respectful, patient‑centered environment.

Qualifications

• Current CNA certification meets state requirements.
• CPR certification up to date.
• Valid driver's license and reliable transportation.
• Preferred CNA experience within last two years.

Responsibilities

• Provides personal care services to patients.
• Assists with ambulation, transfers, ROM, positioning, and household chores.
• Plans and prepares nutritious meals and assists with feeding.
• Performs homemaking and environmental services as assigned.
• Observes, reports and documents changes in patient status and follows care plan.
• Assists patients with prescribed exercises as instructed by nurse or therapist.
• Performs and records simple urine tests for sugar, acetone and albumin.
• Other duties as assigned.
