Job summary

Enhabit Home Health & Hospice seeks a Registered Nurse in a leadership role to coordinate hospice services with physicians, patients, and caregivers. You will review patient information at various care milestones, ensure scheduling with field staff, and maintain strong care coordination and regulatory compliance.

The ideal candidate has 2+ years nursing experience, CPR certification, a valid state license, and proficiency with Microsoft Office and mobile devices.

Qualifications

• Graduate of an approved school of professional nursing.
• Current valid nursing license per state requirements.
• Minimum two years nursing experience; hospice or home health preferred.
• CPR certification maintained.
• Valid state driver license and automobile liability insurance.
• Proficient with Microsoft Office and mobile devices.
• Bilingual English/Spanish preferred for clear communication.

Responsibilities

• Coordinate hospice services to ensure care delivery meets standards and procedures.
• Review and approve patient information at key care milestones.
• Facilitate communication among physicians, referral sources, patients, caregivers, and staff.
• Schedule patients with field clinicians and maintain care coordination and continuity.
