Job summary

Enhabit Home Health & Hospice is seeking a nurse leader to coordinate and optimize home health services with physicians, patients, and staff. You will review patient data, ensure care plans meet standards, and manage scheduling to maintain continuity of care.

The role requires an RN license, CPR, and strong communication skills; home health experience is preferred. In-office attendance is essential to supervise and collaborate with teams across departments.

Qualifications

• Graduate of an approved school of professional nursing.
• Valid clinical license, per state requirements, or compact nurse license.
• Minimum two years nursing experience within the past 36 months.
• Minimum one year of home health experience.
• CPR certification.
• Valid state driver license and automobile liability insurance.
• Basic demonstrated technology skills including mobile devices.
• Experience in management preferred.
• Homecare Homebase (HCHB) experience preferred.

Responsibilities

• Coordinate home health services to patients according to standards and procedures.
• Review and approve patient information at start of care, recertification, resumption of care, or evaluation visit.
• Facilitate relationships between physicians, referral sources, patients, caregivers and staff.
• Schedule patients to field clinicians and maintain care coordination and continuity.
