Job summary

Enhabit Home Health & Hospice seeks a nursing leader to coordinate home health services in Texas. You will review patient data at care milestones, and facilitate collaboration among physicians, referrals, patients, caregivers, and clinicians. 

The position requires a registered nursing license, 2 years of recent nursing experience, and 1 year in home health. In-office attendance is essential to supervise, lead teams, and ensure quality care.

Qualifications

• Graduate of an approved school of professional nursing.
• Valid nursing license per state requirements.
• Two years of nursing experience within the last 36 months.
• Minimum one year of home health experience.
• Knowledge of federal, state and local home care laws and regulations.
• CPR certification and valid driver's license.
• Automobile liability insurance as required by law.
• Basic technology skills including mobile devices and Microsoft Office.
• Preferred management experience.
• Preferred Homecare Homebase experience.
• Preferred bilingual English/Spanish.
• Dependable transportation and ability to drive in all weather.
• Oral and written communication, English fluency, active listening.
• In-office attendance required; no remote work for this role.

Responsibilities

• Coordinate home health services delivered to the patient per standards.
• Review and approve patient information at start of care, recertification, or resumption of care.
• Facilitate relationships with physicians, referrals, patients, caregivers and staff.
• Schedule patients to field clinicians and maintain care coordination and continuity.
