The Care Team Hospice is seeking an experienced Clinical Manager (RN) to lead and support our clinical team, ensuring compassionate, high-quality end-of-life care. As Clinical Manager, you will partner with branch leadership to oversee daily operations, mentor field clinicians, and ensure compliance with hospice regulations and documentation standards.

Qualifications

• Active RN license required.
• Minimum of two years of RN experience, including at least one year in leadership in hospice/home health.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong leadership, clinical assessment, communication, and organizational skills.
• Ability to coach teams, manage multiple priorities, and thrive in a fast-paced healthcare environment.
• Valid driver's license, reliable transportation, and current automobile insurance required.

Responsibilities

• Lead and support daily clinical operations while ensuring compliance with hospice regulations, company policies, and quality standards.
• Mentor, educate, and develop clinical staff through coaching, orientation, documentation review, and ongoing performance support.
• Oversee patient care coordination, care plan reviews, admissions, orders, and interdisciplinary team collaboration to ensure exceptional patient outcomes.
• Monitor quality metrics, conduct chart audits, and support survey readiness and performance improvement initiatives.
• Partner with branch leadership, physicians, referral sources, and operational teams to promote clinical excellence and outstanding patient and family experiences.

The Care Team Hospice is seeking an experienced and compassionate Clinical Manager (RN) to lead and support our clinical team. This is an excellent opportunity for a Registered Nurse who is passionate about developing clinicians, ensuring quality patient care, and helping shape the success of a growing hospice program.

As a Clinical Manager, you'll partner with branch leadership to oversee daily clinical operations, support patient care coordination, mentor field clinicians, and ensure compliance with hospice regulations and documentation standards.
