Job summary

The Care Team Hospice seeks an experienced Registered Nurse to advance into clinical leadership as Clinical Team Leader. You will partner with the Branch Director and Clinical Manager to support day-to-day operations and guide nurses, aides, social workers, chaplains, and other team members in delivering compassionate hospice care.

This role combines hands-on patient care with coaching, documentation improvement, staffing coordination, and quality initiatives.

Qualifications

  • Active and unrestricted RN license in the state of practice.
  • Graduate of an accredited nursing program (ADN or BSN).
  • Minimum two years RN experience within the last 36 months, hospice/home health preferred.

Responsibilities

  • Partner with Branch Director to oversee daily clinical operations and staffing.
  • Provide leadership, coaching, and clinical guidance to nurses and interdisciplinary staff.
  • Review care plans, documentation, admissions, and discharges to ensure quality care.
  • Monitor quality metrics and participate in QAPI activities for improvement.
  • Coordinate with physicians, families, and partners to deliver exceptional patient experience.
  • On-call support and direct patient care as needed to maintain continuity.

As a Clinical Team Leader, you'll work alongside the Clinical Manager and Administrator to support the day-to-day clinical operations of the branch while helping nurses, aides, social workers, chaplains, and other team members deliver exceptional hospice care.This is a great opportunity for an RN who enjoys being the person others turn to for guidance. You'll help clinicians solve problems, strengthen documentation and care planning, coordinate staffing and patient needs, and ensure the team has the support and resources needed to be successful.You'll still use your clinical expertise, but your impact becomes bigger than a single caseload. Our goal is to be the most trusted partner in hospice, supporting patients and their families with comfort, dignity, and respect during life's most important moments.We offer a supportive, growth-focused environment along with a comprehensive benefits package, including:• Medical, Dental, and Vision Insurance, plus Wellness Programs• Career Path Program with opportunities for advancement and increased compensation• Generous Paid Time Off and Paid Holidays• Tuition Reimbursement and Certification Assistance• Mileage Reimbursement• 401(k) with Company Match• Additional Voluntary Coverage• Great Total Rewards Package!!Key Responsibilities• Partner with the Branch Director to oversee daily clinical operations, staffing, patient care coordination, and overall branch clinical performance.• Provide leadership, coaching, education, and clinical guidance to nurses and interdisciplinary team members while helping clinicians grow and succeed.• Review patient care plans, clinical documentation, orders, admissions, discharges, and workflows to promote quality, continuity of care, and regulatory compliance.• Monitor quality and performance through QAPI activities, documentation audits, patient outcomes, and other clinical metrics while identifying opportunities for improvement.• Collaborate with physicians, patients, families, referral partners, and interdisciplinary team members to resolve concerns and ensure an exceptional patient and family experience.• Support clinician onboarding, competency development, performance evaluations, survey readiness, and ongoing education.• Provide direct patient care and on-call support when needed to ensure continuity of care and support the clinical team.Qualifications• Active and unrestricted Registered Nurse (RN) license in the state of practice required.• Graduate of an accredited school of nursing; Associate or Bachelor's Degree in Nursing accepted.• Minimum of two years of Registered Nurse experience within the previous 36 months required.• Hospice or Home Health experience STRONGLY preferred.• Previous supervisory, leadership, charge nurse, or clinical management experience preferred.• Strong clinical judgment with the ability to coach others, prioritize competing needs, and make confident decisions in a fast-paced environment.• Excellent communication, organization, problem-solving, and relationship-building skills.• Current CPR certification required.• Valid driver's license, reliable transportation, and current automobile insurance required.
