Job summary

The Care Team Hospice is seeking an experienced Clinical Manager (RN) to lead and support the clinical team at Winfield Hospice - Lisle, IL. This is a full-time role focused on developing clinicians, ensuring high-quality patient care, and upholding hospice standards.

As Clinical Manager, you will partner with branch leadership to oversee daily operations, mentor staff, coordinate patient care, and drive performance improvement while delivering compassionate end-of-life care.

Qualifications

• Active RN license required.
• Minimum of two years of RN experience, including at least one year of leadership experience in hospice, home health, or a related healthcare setting.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong leadership, clinical assessment, communication, and organizational skills.
• Ability to coach teams, manage multiple priorities, and thrive in a fast-paced healthcare environment.
• Valid driver's license, reliable transportation, and current automobile insurance required.

Responsibilities

• Lead and support daily clinical operations while ensuring compliance with hospice regulations, company policies, and quality standards.
• Mentor, educate, and develop clinical staff through coaching, orientation, documentation review, and ongoing performance support.
• Oversee patient care coordination, care plan reviews, admissions, orders, and interdisciplinary team collaboration to ensure exceptional patient outcomes.
• Monitor quality metrics, conduct chart audits, and support survey readiness and performance improvement initiatives.
• Partner with branch leadership, physicians, referral sources, and operational teams to promote clinical excellence and outstanding patient and family experiences.

The Care Team Hospice is seeking an experienced and compassionate Clinical Manager (RN) to lead and support our clinical team.
