Job summary

The Care Team Hospice is seeking an experienced Clinical Manager (RN) to lead and support our clinical team in Naperville, IL. This role focuses on developing clinicians, ensuring high-quality patient care, and supporting a growing hospice program.

As Clinical Manager, you will partner with branch leadership to oversee daily operations, mentor field clinicians, and ensure regulatory compliance and documentation standards while delivering exceptional end-of-life care to patients and families.

Qualifications

• Active RN license required.
• Minimum of two years RN experience with at least one year in leadership, hospice/home health or related setting.
• Experience with Homecare Homebase (HCHB) preferred.

Responsibilities

• Lead and support daily clinical operations while ensuring compliance with hospice regulations and quality standards.
• Mentor, educate, and develop clinical staff through coaching, orientation, documentation review, and ongoing performance support.
• Oversee patient care coordination, care plan reviews, admissions, orders, and interdisciplinary team collaboration to ensure exceptional patient outcomes.
• Monitor quality metrics, conduct chart audits, and support survey readiness and performance improvement initiatives.
• Partner with branch leadership, physicians, referral sources, and operational teams to promote clinical excellence and outstanding patient/family experiences.

The Care Team Hospice is seeking an experienced and compassionate Clinical Manager (RN) to lead and support our clinical team. This is an excellent opportunity for a Registered Nurse who is passionate about developing clinicians, ensuring quality patient care, and helping shape the success of a growing hospice program.
