Job summary

Affinity Health Management is seeking a Clinical Care Coordinator to provide administrative support to the Clinical Care team and manage the electronic medical records system from its Port Charlotte, FL office. The role also supports back-office functions including billing, payroll, human resources and intake.

The successful candidate will train new Clinical Care Coordinators and assist with ongoing regional support while maintaining PHI confidentiality and ensuring compliance across tasks.

Qualifications

• High School Diploma (College Degree Preferred)
• At least two years health care experience, preferably in hospice
• Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities
• HCHB/PointClickCare knowledge is preferred
• Proficient in MS Office and Google Applications

Responsibilities

• Accurately enters a variety of data into various information systems
• Executes workflow functions in electronic medical record system
• Verifies accuracy of data entered through collaboration with Clinical Manager(s) and/or Clinical Director
• Manages incoming, outgoing and interoffice mail
• Provides customer support to patients and colleagues via phone, email and fax
• Alerts appropriate management team members regarding potential compliance issues
• Maintains confidentiality of PHI and organization information at all times
• Order DME and alerts DME supplier when equipment is no longer in use/needs to be exchanged for other, etc.
• Participates and scribes IDG meetings
• Performs other specific projects relating to office procedures
• Provides training and ongoing support to new Clinical Care Coordinators within the region
