Qualifications

• Ongoing commitment to education and obtaining CPE; CPR certification required.

Responsibilities

• Assesses spiritual needs of patients and families and ensures timely services.
• Develops and participates in plans of care to meet identified spiritual needs.
• Provides direct spiritual care to patients and families/caregivers.
• Liaises with community chaplains and spiritual counselors; maintains records of services.
• Documents services and communicates with chaplains and spiritual counselors.
• Participates in interdisciplinary meetings and supports the care team on spiritual care aspects.
