Job summary

The Chaplain supports patients through the dying process, grief, and bereavement, coordinates with clergy as requested, and maintains up-to-date care plans reflecting evolving needs and goals.

Qualifications

• High school diploma or GED required; Bachelor's preferred unless state regulation requires.
• Completion of at least one unit of CPE preferred.
• Minimum of one year experience providing spiritual/pastoral support.
• Experience in loss, bereavement, or trauma counseling preferred.

Responsibilities

• Explains hospice services and Medicare benefits to patients and families; obtains informed consent and documents as requested.
• Submits documentation to facilities after patient visits; paperwork completed timely per policy.
• Articulates the regulatory framework governing spiritual support (federal/state).
• Participates as a member of the IDT and contributes to the plan of care.
• Provides spiritual care to patient and family; supports involvement of community clergy as requested.
• Coordinates with IDT to ensure appropriate, continuous spiritual care.
