Job summary

Providence at Home with Compassus seeks a Chaplain to provide spiritual support to patients and families in a hospice setting. The role upholds Compassus values and works within the interdisciplinary team to deliver compassionate, respectful care that honors diverse beliefs and bereavement needs.

TheChaplain will explain hospice and Medicare benefits, coordinate with IDT, and counsel families through grief and transitions, while adhering to regulatory and ethical standards.

Qualifications

• High school diploma or GED required.
• Bachelor's degree in Divinity, Social Sciences, or related field preferred.
• Minimum of one year of experience in spiritual/pastoral support.
• Completion of at least one unit of CPE preferred.

Responsibilities

• Explains hospice services and Medicare benefits to patients and families; obtains Informed Consent and Election of Benefits documents as requested.
• Submits appropriate documentation to facilities at the completion of patient visits; documentation is completed and submitted timely according to policy.
• Articulates the regulatory framework governing the role of the Spiritual Counselor (federal and state).
• Participates as a member of the IDT, including development and implementation of the plan of care.
• Provides spiritual care support to the patient and family; supports involvement of community clergy as requested.
• Collaborates with IDT to coordinate spiritual care for patient and family to ensure continuity and quality of care.
• Maintains updated, comprehensive care plans reflecting current problems, goals and interventions.
