Job summary

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with license in good standing.
• 1 year of nursing experience within last 3 years, or BSN +2 years in health/home/hospice preferred.
• Supervisory experience not required; may supervise hospice aides.
• Criminal background checks required after offer.
• Excellent observation, verbal/written communication, problem solving and math skills.
• Must be able to lift and move patients; valid driving license with insured vehicle.
• CPR certification required (hands-on verification preferred).

Responsibilities

• Completes initial and ongoing patient/family assessment to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Writes and revises plan of care; re-evaluates patient/family needs.
• Initiates and updates plan of care as patient status changes.
• Develops goals in care plan based on nursing diagnosis and pallaitive actions.
• Delivers medications and treatments as prescribed; educates patient/family.
• Acts as case manager and coordinates care for assigned caseload.
• Maintains accurate clinical notes and communicates with physician.
• Ensures safe environment and continuity of care with the team.
• Participates in on-call duties and inter-disciplinary group meetings.
