Job summary

Continuum Care of Washington is seeking a dedicated Registered Nurse for hospice home care in the Renton area. You will plan, coordinate and deliver nursing services to patients and families in their homes, utilizing the nursing process and collaborating with the interdisciplinary team.

Responsibilities include assessments, care planning, medication administration, patient education, and case management to ensure comfort and quality end-of-life care across the community setting.

Qualifications

• Graduate of an accredited school of nursing.
• Registered nurse with license in good standing.
• Minimum of 1 year nursing experience in last 3 years; or BSN with 2 years’ experience in public health, home care or hospice preferred.
• Management experience not required.
• Satisfactory references and criminal background checks required.
• CPR certification required.

Responsibilities

• Completes initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Writes and initiates plan of care, revises as needed based on patient status.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family in meeting nursing and related needs.
• Acts as Case Manager when assigned and coordinates patient care for caseload.
• Maintains accurate clinical notes and communicates with physician and team.
• Teaches patient and family self-care and health instructions.
