Continuum-Care-of-Washington is seeking a Registered Nurse to plan, organize and direct hospice care in home and community settings. You will conduct comprehensive assessments and develop patient-centered care plans with the interdisciplinary team.

The role emphasizes education, care coordination, and symptom management, with responsibilities including on-call duties, documentation, and guiding families through care decisions.

Qualifikationen

• Graduate of an accredited school of nursing with RN licensure.
• Current CPR certification and ability to use nursing process.
• Strong assessment, planning and patient/family education skills.
• Experience in home health, hospice or community health preferred.
• Valid driver’s license and reliable vehicle for community visits.

Aufgaben

• Perform initial and ongoing comprehensive patient and family assessments.
• Develop and revise care plans based on nursing diagnoses.
• Administer prescribed medications and treatments; monitor outcomes.
• Coordinate care with physicians, family and interdisciplinary team.
• Provide health education and emotional support to patients and families.
• Document accurately and maintain compliance with regulations.
• Participate in on-call duties and ensure safe home environments.
• Supervise aides when assigned and contribute to quality improvement.
• Promote continuity of care across transitions and settings.
• Maintain professional growth through continuing education.
