Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager to oversee the Plan of Care for hospice patients in Olympia, WA. You will evaluate care, collaborate with the care team, and implement strategies to promote physical, psychosocial, emotional, and spiritual well-being.

Responsibilities include conducting initial hospice eligibility assessments within 48 hours, coordinating the plan of care, documenting progress, and supporting patients and families.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current CPR Certification.
• Valid driver's license and access to reliable transportation.
• Ability to function in any home situation regardless of client demographics.

Responsibilities

• Perform initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify patient/family needs and re-assess at least every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family responses.
• Collaborate with patient/family, attending physician and IDG members daily.
• Instruct and supervise patient/family in self-care techniques when appropriate.
