Job summary

Shalom Hospice of Washington seeks an experienced Registered Nurse to plan, direct and deliver home-based hospice care in Washington. You will assess patients, develop care plans with families, administer medications, and coordinate care with an interdisciplinary team.

You must be licensed as an RN in good standing, have strong communication and clinical skills, and be able to work independently in home settings. A valid driver license and reliable vehicle are required to visit patients.

Qualifications

• Graduated from an accredited school of nursing.
• Registered Nurse license in good standing to practice.
• Minimum 1 year of professional nursing experience in the last 3 years; or a BSN with 2+ years in public health, home care, or hospice.
• Management experience not required; may supervise hospice aides.
• Offer contingent upon references and background checks as required.
• Excellent observation, verbal and written communication, problem solving and basic math skills.

Responsibilities

• Complete initial and ongoing comprehensive patient and family assessments.
• Provide professional nursing care using the nursing process.
• Develop and revise care plans with patient and family involvement.
• Administer medications and treatments as prescribed by the physician.
• Coordinate patient care as Case Manager when assigned.
• Educate patient and family; ensure safe environment and continuity of care.
• Participate in on-call duties and interdisciplinary team meetings.
