Job summary

Shalom Hospice is seeking a Registered Nurse to lead in-home hospice care, performing comprehensive patient assessments and developing collaborative care plans with families. You will coordinate with physicians and a multidisciplinary team to ensure comfort and quality of life for patients in their homes.

The role emphasizes patient education, care coordination, and adherence to nursing standards while supporting families, with a need for strong communication and clinical skills.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with a license in good standing to practice.
• Minimum of 1 year of professional nursing experience within the last 3 years (or BSN with 2+ years in public health/home care/hospice).
• CPR certification (may require hands-on).
• Valid driver’s license and reliable automobile.

Responsibilities

• Perform initial and ongoing comprehensive patient/family assessments.
• Provide professional nursing care following the nursing process.
• Develop and revise plans of care with patient/family involvement.
• Coordinate patient care with the interdisciplinary team and supervise aides.
