Job summary

Shalom Hospice seeks a compassionate Registered Nurse to plan, organize, and direct hospice care in patients' homes and communities. You will base care on nursing assessments, develop personalized plans, and coordinate with the interdisciplinary team.

Responsibilities include on-going assessments, plan of care initiation/adjustment, patient and family education, and on-call duties. Strong communication and home care experience preferred; valid RN license required.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN in good standing.
• Minimum 1 year of nursing experience within last 3 years; or BSN with 2+ years in public health/home care/hospice preferred.
• Management experience not required; may supervise hospice aides.
• Contingent upon references and background checks.

Responsibilities

• Complete initial and ongoing comprehensive patient/family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care as patient status changes.
• Initiate and adjust the plan of care with physician input.
• Record clinical notes and report changes to the physician.
• Coordinate care with interdisciplinary team and community resources.
• Educate patient and family on self-care, medications, and care plans.
• Ensure a safe environment and continuity of care across shifts.
• Participate in on-call duties and quality improvement activities.
