Job summary

Affinity Health Management is seeking a Registered Nurse experienced in hospice care to plan, organize, and direct community-based services for patients and families at home. The role emphasizes community health education and coordinating care with an interdisciplinary team.

The RN will assess needs, develop and revise care plans, administer medications, counsel families, and ensure a safe, supportive home environment while meeting professional nursing standards.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse, with a License in good standing to practice.
• Minimum of 1 year of experience as a professional nurse in the last 3 years, or a BSN with 2+ years in public health/home care/hospice.

Responsibilities

• Complete initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and implement care plans with patient and family involvement.
• Administer medications and treatments as prescribed by the physician.
• Counsel patient and family in nursing and related needs.
• Coordinate patient care as Case Manager when assigned.
• Participate in interdisciplinary team meetings.
