Affinity Health Management is seeking an experienced registered nurse to direct hospice care in patients' homes and communities. You will assess patients, plan and direct care, administer medications, and coordinate services with families and the care team.

The role emphasizes community health education, ongoing assessment, and collaboration with interdisciplinary staff to meet patient needs and promote comfort and quality of life.

Qualifikationen

• Graduate of an accredited school of nursing.
• Registered Nurse with a license in good standing to practice.
• One year of professional nursing experience in the last three years; or a BSN with two years’ experience in public health/home care/hospice preferred.

Aufgaben

• Complete initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care with patient/family involvement and goals.
• Administer medications and treatments as prescribed by the physician.
• Counsel patients and families on nursing needs and self-care techniques.
• Coordinate patient care as a case manager when assigned by the Clinical Supervisor.
• Maintain accurate clinical notes and communicate changes with the physician and care team.
• Participate in on-call duties and ensure equipment and services are available.
