Job summary

Affinity Hospice Management LLC seeks a registered nurse to plan, organize and direct hospice care in patients\' homes and communities, leveraging community health resources. Responsibilities include initial and ongoing assessments, developing a care plan, administering prescribed medications, coordinating with physicians and the interdisciplinary team, and supporting patients and families.

Qualifications

Graduate of an accredited school of nursing.
Active Registered Nurse license in good standing.
Minimum one year recent professional nursing experience; hospice/home health preferred.
Must have valid driver's license and automobile.
CPR certification required (hands-on verification).
Excellent observation, verbal and written communication skills.

Responsibilities

Complete initial and ongoing patient/family assessments to determine hospice needs.
Develop and revise care plans based on nursing diagnoses.
Administer medications and treatments as prescribed by physician.
Counsel patients and families; provide health instructions and self-care guidance.
Act as Case Manager and coordinate care with the interdisciplinary team.
