Job summary

Affinity Hospice seeks an experienced Registered Nurse Case Manager to plan, organize, and direct hospice care in patients’ homes and communities. The role emphasizes community health education and coordinating an interdisciplinary team for each patient’s plan of care.

The ideal candidate has an RN license, CPR certification, and 2+ years in clinical care, home health, or hospice. A valid driver’s license and reliable car are required.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse (RN).
• Current CPR certification.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement/develop/document the patient plan of care.
• Provide care utilizing infection control measures and OSHA guidelines.
• Monitor assigned cases for third‑party payer compliance.
• Supervise hospice aides and licensed practical nurses.
• Perform other nursing duties as assigned by management.
