Job summary

Affinity Health Management is seeking a registered nurse to plan, organize and direct hospice care in home and community settings. The RN will build on community resources to meet the needs of patients and families, guiding care across the care continuum.

The role emphasizes comprehensive patient assessment, care planning, patient/family education, and collaboration with the interdisciplinary team to ensure compassionate, quality end-of-life care. Valid RN license and driving access required.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license in good standing.
• Experience in home health, hospice, or public health preferred.
• Valid driver's license and reliable vehicle.
• CPR certification required.
• Ability to lift and move patients; strong communication skills.

Responsibilities

• Conduct comprehensive patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise care plans with patient and family involvement.
• Coordinate patient care as Case Manager when assigned.
• Educate patients and families on self-care and health maintenance.
• Maintain accurate clinical notes and communicate with physicians.
• Participate in on-call duties and ensure necessary equipment and services are arranged.
• Supervise hospice aides when required.
