Enhabit Home Health & Hospice is seeking a field-based professional to support transitional care across patients and providers. You will represent the branch to patients, families, physicians, hospitals, and community groups, promoting Enhabit services and coordinating care.

Responsibilities include developing patient-centered transition plans, integrating clinical guidelines, and educating the public about available home health services.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must understand federal, state, and local laws and regulatory guidelines governing hospice operations.
• Must have basic technology skills, including operation of a mobile device.
• Hospice knowledge is preferred.

Responsabilités

• Represent the branch in external transitional care activities with patients, families, physicians, hospitals, facilities, and health groups to promote Enhabit services.
• Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency.
• Serve as a public awareness representative and educate related to provided services.

Formation

Graduate of an approved school of clinical education

Licensed in the state where they currently practice

At least one year home care experience

Understanding of federal, state, and local laws and regulatory guidelines governing hospice operations

Tech skills including operation of a mobile device

Hospice knowledge preferred
