Job summary

Enhabit Home Health & Hospice seeks a compassionate professional to serve as a branch representative, guiding patients and families through transitional care services. You will collaborate with providers, educate about Enhabit's home-based programs, and promote high-quality, patient-centered care across the community.

Requirements include an approved clinical education program graduate, state licensure, at least one year of home health experience, and familiarity with relevant laws and

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must have demonstrated knowledge of federal, state, and local laws and regulatory guidelines governing hospice operations.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations and similar health groups to apprise them of Enhabit Home Health & Hospice services.
• Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency.
• Serve as a public awareness representative and educate regarding provided services.
