Job summary

Enhabit Home Health & Hospice is seeking a dedicated professional to support transitional care across our service areas. You will engage with patients, families, physicians, hospitals and community providers to promote Enhabit services and coordinate care.

The role emphasizes developing patient-centered plans, complying with regulatory guidelines, and educating the public about home health and hospice offerings.

Qualifications

• Graduate of an approved school of clinical education.
• Licensed in the state where they currently practice.
• At least one year experience in home care.
• Knowledge of federal, state, and local laws and regulatory guidelines governing hospice.
• Basic technology skills including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups and institutions.
• Develop patient-centered transition plans that promote quality and efficiency by integrating clinical guidelines and protocols.
• Serve as a public awareness representative and educate about the provided services.
