Qualifications

• Education and experience, essential: graduate of approved school of clinical education and at least one year in home care.
• Must understand federal, state, and local laws and regulatory guidelines governing hospice operations.
• Basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and professional contacts to promote Enhabit services.
• Integrate clinical guidelines and metrics to develop patient-centered transition plans that promote quality and efficiency.
• Serve as a public awareness representative and provide education related to Enhabit services.
