Job summary

Enhabit Home Health & Hospice is seeking a branch representative to engage with patients, families, physicians, hospitals and senior living communities to inform them about Enhabit services. You will apply clinical guidelines, protocols and other metrics to develop patient-centered transition plans that improve quality and efficiency.

Additionally, you will act as a public awareness advocate and provide education on the services we offer, supporting growth of the branch while upholding

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of a hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities, and in professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups and institutions, to apprise them of the availability of Enhabit Home Health & Hospice services.
• Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency.
• Serve as a public awareness representative and will be responsible for education related to provided services.
