Job summary

Affinity Hospice Management LLC is seeking an experienced Registered Nurse to plan, organize and direct hospice care in home and community settings in Florida. The role emphasizes patient/family assessment, care planning, and coordination with an interdisciplinary team.

The ideal candidate has RN licensure in good standing and 1+ year of nursing experience in the last 3 years, with preference for home care or hospice.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with license in good standing to practice.
• Minimum 1 year of nursing experience in last 3 years; or BSN with at least 2 years’ experience in public health/home care/hospice preferred.
• Supervisory experience not required; may supervise hospice aides.
• Criminal background checks required after offer.

Responsibilities

• Complete initial and ongoing comprehensive assessment of patient and family.
• Provide professional nursing care using nursing process.
• Write and initiate plan of care; reassess and revise as needed.
• Administer medications and treatments as prescribed.
• Counsel patient and family; provide health instructions and self-care guidance.
• Act as case manager when assigned; coordinate patient care.
• Maintain accurate clinical notes and communicate with physicians.
• Ensure safe environment and continuity of care with the team.
• Participate in on-call duties and quality improvement activities.
