Job summary

Affinity Hospice Management LLC seeks a Registered Nurse to direct hospice care in home and community settings in Florida. You will assess patients, implement and revise care plans, administer treatments, and counsel families on nursing needs.

The role requires a Florida RN license, a current CPR certification, and 1+ year of professional nursing experience or a BSN with 2+ years in health/home care. A driver’s license is essential for this on-site position in North Port.

Qualifications

Graduate of an accredited nursing program with Florida license.
Minimum one year of professional nursing in the last three years, or a BSN with two years’ experience (preferably in public health/home care/hospice).
Valid driver’s license and vehicle in good working order; CPR certification required.

Responsibilities

Perform comprehensive patient and family assessments to determine hospice needs.
Deliver professional nursing care using the nursing process in home and community settings.
Develop and revise individual care plans with patient and family involvement.
Administer medications and treatments as prescribed and monitor outcomes.
Counsel patients and families on nursing needs and self-care at home.
Coordinate patient care as Case Manager when assigned by Clinical Supervisor.
Communicate with physicians and inter-disciplinary team about patient status.
