Job summary

Affinity Hospice Management LLC is seeking a registered nurse to lead in-home hospice care, performing comprehensive patient assessments and coordinating care with families and the interdisciplinary team.

The role emphasizes planning, implementing, and evaluating individualized care plans, administering prescribed treatments, and maintaining meticulous documentation in accordance with Florida regulations.

Qualifications

• Graduate of an accredited school of nursing.
• RN license in Florida.
• 1+ year experience preferred in home hospice Nursing is preferred.

Responsibilities

• Perform comprehensive patient and family assessments.
• Develop and revise plan of care with the interdisciplinary team.
• Administer medications and treatments per physician orders.
• Coordinate with physicians and caregivers to ensure continuity of care.
• Participate in on-call duties and weekend coverage.
• Maintain accurate clinical documentation and progress notes.
• Educate patients and families on disease management and home care.
