Job summary

Gentiva Hospice is seeking a Hospice Care Consultant (HCC) to lead strategic sales efforts and build lasting relationships with referral partners in the healthcare community. This full-time role emphasizes educating providers about hospice options and driving appropriate referrals.

You will collaborate with clinical and operations teams to align services with patient needs, manage conversations with hospitals, home health agencies, SNFs, clinics, and physician offices, and contribute to growth

Qualifications

• Bachelor's degree or equivalent experience
• Minimum 2 years in healthcare sales, business development, or referral marketing
• Strong relationships with physicians, hospitals, SNFs or home health professionals
• Experience in hospice, palliative care, or home health is a plus
• Excellent communication, presentation, and time-management skills
• Proficiency in Microsoft Office Suite and CRM platforms
• Sensitivity to the needs of terminally ill patients and their families
• Proven track record of meeting or exceeding admissions and census targets

Responsibilities

• Educate, inform, and inspire confidence in our services so that the right patients are referred at the right time
• Identify new referral opportunities by calling on hospitals, home health agencies, SNFs, assisted living communities, clinics, and physician offices
• Engage in meaningful conversations with healthcare professionals about patient needs and how hospice care can support quality of life
• Plan and execute strategic marketing initiatives to increase awareness and drive referral growth
• Represent our organization at community events, professional associations, and educational in-services to promote our services
• Serve as a resource and advocate for patients and families, ensuring they understand hospice options and the benefits of early referrals
