AccentCare in Wisconsin is seeking a Hospice Care Consultant to connect patients and families with compassionate hospice services. You will educate referral sources, build relationships with physicians, hospitals, SNFs, and community partners, and drive timely admission.

The role emphasizes compassionate patient-centered care, staff collaboration with Admissions and Marketing, and ongoing training to meet ethical standards and quality goals.

Qualificações

• Bachelor's degree or equivalent experience.
• 5 years proven experience in home health, healthcare sales, or hospice sales.
• Strong communication and relationship-building skills.

Responsabilidades

• Develop and maintain trusting relationships with physicians, hospitals, SNFs, and community organizations through in-person visits and educational conversations.
• Drive hospice sales by nurturing current relationships, identifying new referral opportunities and increasing awareness of our programs.
• Conduct presentations and in-services to educate partners on hospice benefits and eligibility.
• Collaborate with Admissions and Marketing teams to develop and carry out community outreach plans that reflect our mission of honoring life and offering hope.
• Partner with clinical staff to ensure referrals transition smoothly into care with patient comfort and dignity in mind.
• Uphold commitments to quality, ethics, and learning by participating in training and honoring confidentiality.
