Job summary

AccentCare is seeking a Hospice Care Consultant to expand our referral network and educate partners about hospice benefits. You will cultivate relationships with physicians, hospitals, and community organizations, driving hospice sales while ensuring timely, high-quality care for patients and families.

The role emphasizes independent territory management, strong communication, and collaboration with Admissions and Marketing to execute outreach strategies in the Rosemont, IL area.

Qualifications

• Bachelor's degree and/or 5 years proven experience in home health, healthcare sales, or hospice sales.
• Strong communication, problem-solving and relationship-building skills.
• Ability to work independently and manage a territory effectively.
• Knowledge of hospice regulations and services preferred.

Responsibilities

• Develop and maintain relationships with physicians, hospitals, SNFs, and community organizations through in-person visits and education.
• Drive hospice sales by nurturing current relationships and identifying new referral opportunities.
• Conduct presentations and in-services to educate partners on hospice benefits and eligibility.
• Collaborate with Admissions and Marketing to develop and execute community outreach plans aligned with our mission.
• Partner with clinical staff to ensure referrals transition smoothly into care, prioritizing patient comfort and family support.
• Participate in training and planning outreach strategies while maintaining confidentiality.
