Job summary

AccentCare seeks a Hospice Care Consultant to connect providers and community partners with compassionate hospice services in Snohomish County, WA. This role focuses on hospice sales and educating referral sources to ensure timely, quality patient care.

You will build relationships with physicians, hospitals, SNFs, and community groups, drive referrals, present hospice benefits, and collaborate with Admissions and Marketing to implement outreach plans.

Qualifications

• Bachelor’s degree or 5+ years in home health, healthcare sales, or hospice sales.
• Strong communication, problem-solving and relationship-building skills.
• Ability to work independently and manage a territory effectively.

Responsibilities

• Develop and maintain relationships with physicians, hospitals, SNFs, and community organizations through in-person visits and educational conversations.
• Drive hospice sales by nurturing current relationships, identifying new referral opportunities and increasing program awareness.
• Conduct presentations and in-services to educate partners on hospice benefits and eligibility.
• Collaborate with Admissions and Marketing to develop and execute community outreach plans aligned with our mission.
• Partner with clinical staff to ensure referrals transition smoothly into care, prioritizing patient comfort and family support.
• Participate in training and planning to maintain quality and confidentiality.
