Job summary

Advocate Aurora Health at Home is seeking an experienced Registered Nurse to coordinate hospice and home health services. You will educate patients on service lines, assist with admission processes, and ensure regulatory compliance across the care continuum.

Responsibilities include collaboration with physicians and care managers, attending huddles and rounds, and supporting discharge planning to meet patient needs in the home setting.

Qualifications

• License/Registration/Certification: Registered Nurse
• a valid driver’s license issued by the Division of Motor Vehicles
• Issued by (Governing Body): State in which the team member practices
• Level of Education: Graduate from an accredited school of nursing, BSN preferred
• Field of Study (if applicable): Nursing
• Years of Experience: Typically requires 2 years of clinical nursing experience with discharge planning and/or care management
• Describe Type Experience: Hospice experience preferred

Responsibilities

• Collaborates with physicians, nurses, social workers and care managers to facilitate smooth patient transitions
• Attends huddles and discharge rounds and serves as a resource for home-based services
• Educates patients on agency service lines to ensure hospice eligibility and regulatory compliance
• Collects and records information to initiate Advocate Aurora Health at Home services
• Educates patients and providers on financial coverage for services
• May follow hospice patients readmitted to hospital or post-acute setting
• Provides age-appropriate care and participates in potential precepting
• Participates in admission processes for hospice services
• Collaborates with business development and product line director to grow admissions
