Qualifications

• Graduate from an accredited nursing program.
• Georgia RN license required.
• Minimum 2 years acute care RN experience.
• BLS certification required; instructor preferred.
• Driver's license with private vehicle required.

Responsibilities

• Introduces hospice care and services to the patient and family.
• Evaluates goals for care and readiness to adopt a palliative approach.
• Reviews medical records for disease progression and care needs.
• Collaborates with physicians and hospital staff on admission and care levels.
• Coordinates hospice admission and discharge planning with care teams.
• Orders medications, equipment and supplies for home care.
• Educates referral sources and hospital staff about hospice services.
• Supports family through anticipatory grief and respects care preferences.
