Job summary

Wellstar Health System seeks an RN hospice liaison to educate patients and families about hospice options, coordinate admissions, and ensure smooth transitions from hospital to home or facility settings.

The role is part-time, about 20 hours per week with weekend coverage at Kennestone campus. Active RN license with 2 years acute care experience and strong collaboration with physicians and care teams are required.

Qualifications

• Graduate of an accredited nursing program; RN licensure required and BSN preferred.
• RN license (single state) with multi-state compact eligibility.
• Minimum 2 years of acute care nursing experience.
• Strong interpersonal and collaborative skills, and effective communication in English.

Responsibilities

• Introduce hospice care and services to patients and families.
• Evaluate goals for care and readiness for palliative approaches.
• Coordinate admission processes and hospice referrals with physicians and care teams.
• Manage medication, equipment, and supplies for hospice patients.
• Educate referral sources and hospital staff on hospice care models.
• Support families through anticipatory grief and end-of-life decisions.
