Job summary

Wellstar Health System in Georgia is seeking a Part-time RN Hospice Liaison to join our patient services team. You will work 20 hours per week, primarily weekends at the Kennestone campus, collaborating with physicians, care coordinators, and community partners, while upholding Georgia

Qualifications

• Graduate of an accredited/approved school of nursing.
• RN licensure in Georgia (or multi-state) as applicable; BLS and license requirements listed in job posting.
• Minimum 2 years of acute care RN experience preferred.
• Strong English communication, interpersonal and organizational skills.

Responsibilities

• Introduce hospice care and services to patient and family.
• Assess care goals, directives, and readiness for palliative approaches.
• Coordinate hospice admission, including orders and transport arrangements.
• Collaborate with physicians, care coordinators, and hospital staff to ensure smooth transitions.
