Wellstar Health System is seeking a part-time RN hospice liaison to support hospice education and referrals across hospital and community settings. Responsibilities include patient assessment, admission coordination, and collaboration with physicians and care teams to ensure a smooth transition to hospice services.

The role requires an active Georgia RN license, BLS certification, and acute care nursing experience.

Qualifications

• RN license in Georgia is required
• Basic Life Support (BLS) certification required
• BLS Instructor certification and BLS Provisional status are preferred
• Valid Georgia Driver's License and private vehicle insurance are required
• Minimum 2 years of acute care nursing experience is preferred

Responsabilités

• Introduce hospice care and services to patient and family
• Evaluate goals for care and readiness for palliative approach
• Review medical records for disease progression and decline
• Assess patient condition and identify needs
• Discuss advance directives and code status with patients and families
• Coordinate hospice admission with physician and hospice intake
• Arrange transportation and prepare admission orders and medications
• Educate referral sources and hospital staff about hospice care and models of care
• Support anticipatory grief and involve patient/family in the plan of care
