• Introduce hospice care to patient and family.
• Assess goals for care and readiness for palliative care.
• Coordinate admission and level of care (GIP, Respite, Routine Home Care).
• Collaborate with physicians, care coordinators, and hospital staff.
• Arrange transportation and educate referral sources about hospice services.
